Before You Make a Request

Please read the following statements before filling out an Event Request Form. If you understand these statements and agree to follow the policies, click the button below to complete the event form.

• Calvary Church operates its facilities in a way consistent with its Statements of Faith.

• The User agrees not to use the premises for any purpose that is contrary to the mission, purpose, or belief of Calvary Church, a biblically based religious institution.

• Calvary Church reserves the right to prohibit the use of its facilities by an individual, group, or organization that conflicts with the church’s beliefs.

• Calvary Church is considered private property, although it is open to the public for worship services, community events, and ministry use.

• The User shall comply with all church policies, rules, and regulations.

• The sale, possession, consumption, and use of alcoholic beverages and illegal drugs are prohibited on all Church properties.

• All Event Requests are reviewed, and facility use is evaluated before determining availability. Requests are not guaranteed.

• A separate Facility Use Agreement must be submitted upon approval of the event.

• All fees assessed are used to offset the cost of staffing, facility, and resource use.
