Calvary Event Request Form
Hello Dream Team! We’re so excited that you want to host an event and are looking forward to working alongside you!
The purpose of this form is to help you be aware of all Calvary has to offer for your event and to assist your ministry in thinking through logistics. Calvary’s team is available to partner with you for your event’s success.
To better help resource our events, Calvary has organized events into three tiers. Each tier has a different set of criteria based on size, intended audience, and the number of needed resources.
Prior to filling out your event form, it will be helpful for you to determine what tier your event falls under so we can best assist you. You can learn more about the three tiers and their requirements below:

A large event intended to invite the whole church AND the community. Tier 1 events are almost exclusively for community outreach and seasonal productions (e.g. Light the Night Fall Festival, Christmas Production, Easter Production, etc.).
Tier 1 events can receive 7 weeks of website/Calvary app promo at minimum.
**Form must be submitted 5 months prior to event.

An event that is available for most, if not all, of the congregation (e.g. Baptisms, Starting Point, Partner Interest Meetings, Open House, Community Serve Day, Worship/Prayer nights, etc.).
Tier 2 events can receive 4-6 weeks of website/Calvary app promo at minimum.
**Form must be submitted 4 months prior to event.

An event intended for a smaller demographic of the congregation or for a specific ministry/ministry's volunteers (e.g. kids, men, women, or a specific ministry like Worship, CARE, etc.).
Tier 3 events can receive up to 4 weeks of website/Calvary app promo at minimum.
**Form must be submitted 3 months prior to event.
Please carefully read the following before filling out this form:
An event request form must be submitted within the required timeframe prior to the event date to allow proper promotional and logistical planning. An event that is submitted after this window of time may not be accepted.
TIER 1: 5 months prior to the event
TIER 2: 4 months prior to the event
TIER 3: 3 months prior to the event
Submitting this form does not mean your request is approved. You will receive event approval confirmation via email within 5 business days of submission.
If you have any graphics needs, it is required to have these graphics made through our Creative Media department due to a need for church branding consistency, multiple graphic size requirements for our website and social media, etc. Graphics created by an external party will not be accepted. You can ask for graphics in the form!
